Insufficient assistance at home, after being discharged, is one of the primary causes of re-admissions to hospital. Increased fatigue and pain can be caused from the alterations in their physical condition, so the support a person receives in this time is crucial. Assistance with daily tasks such as shopping, personal care, medication and appointments may be needed within this time, and that is exactly what Unique's team of health and well-being staff are here for.
We like to encourage independence and the regaining of strength and confidence of self care tasks so your loved one can re-learn how to complete them. We aim to enable your loved one to live at home with minimal or no support, this way they can maintain independence and avoid future readmission to hospital.
Through gentle encouragement we like to work on regaining your loved ones confidence as we recognise spending a period of time in hospital can have a detrimental effect on their self confidence and independence. We aim to make them feel safe and secure in their own home.
